We are in network with most major health insurance plans. While we will attempt to verify your insurance benefits prior to your appointment, it is your responsibility to provide accurate, up-to-date insurance information. We highly recommend that you contact your insurance company prior to your appointment to verify that our practice is contracted with your specific plan within their network.

Important HMO Plan Information:
Please remember that if you have an HMO insurance plan, your insurance company requires that you secure a referral from your primary care physician and/or an insurance authorization in order to proceed with your dermatology appointment. It is your responsibility as a patient to confirm that these necessary steps required by your plan have been completed prior to your appointment. If these tasks are not completed before your appointment, we encourage you to reschedule your appointment with us in advance. You do not need a referral if you are using a PPO plan, are self-pay, or are only being seen for a cosmetic consult or cosmetic services.
Financial Policy
Credit card on file is required for all patients. It is only used to administer patient refunds, collect delinquent payments, and/or apply administrative fees like no-show/late cancellation fees.
Full payment based on a cost estimate is due at the time of your appointment for services performed. This may include co-pay, co-insurance and/or unmet deductible depending on your insurance plan's benefits. Understanding your insurance benefits is important to determine if you will owe payment at the time of service. You will be refunded when indicated once your insurance has been processed.
We accept payments via cash, debit card, or credit card (Visa, Mastercard, Discover, and Amex)
Rescheduling/Cancellation Policy
We kindly request that if you need to reschedule or cancel your appointment, you provide us with a minimum of 24 hours advance notice. This is important in order to improve access to care for all of our patients. Failure to contact us with this required notice will result in a missed appointment fee of $75 for medical appointments and $150 for cosmetic or surgical appointments.
Illness Policy
Please contact our office immediately if you need to reschedule your appointment due to illness.
If you have a runny nose, cough, sore throat, congestion, or sneezing, please wear a mask to your appointment. If you appear ill or have a fever, you may be asked to reschedule.
Required Paperwork Before Your Appointment
Please fill out your online check-in paperwork and sign the consent forms through the Sadio patient portal. If you do your paperwork in advance, your arrival time is 10 minutes prior to your appointment.
If you prefer to complete check-in paperwork at the office, please arrive 20 minutes before your appointment to ensure that paperwork completion does not take away from your appointment time.
Bring your insurance cards, ID, and method of payment to your appointment.
Tips For Your Visit
A dermatology exam requires visualization of the affected areas. Plan to remove clothing or accessories (such as hats, glasses, jewelry, watches, socks/shoes) covering an area of concern. This includes:
Scalp: do not wear hairpieces or hair accessories
Face: do not wear make-up or tinted sunscreen/moisturizer
Nails: do not wear nail polish or other nail adornments
Skin cancer screening appointments: do not wear any of the above items and plan to completely disrobe for a gown exam
